The process in this programme has not come about as a "happy accident"

The first six months of 2020 was spent analysing my process and data from work I had carried out with patients individually since 2005, all of whom had started out with either a Tinnitus Functional Index (TFI) category 4 (54-72%) or 5 (73-100%). These patients also had anxiety either at moderate or severe levels. From the analysis, I was able to translate the process in to what became the 12 Stage Cognitive Behaviour Therapy for Tinnitus E-Programme in 2020, then CBT4T in 2024.

Appointments

The way appointments are included has changed in the 2025-26 version of CBT4T.

Up to October 2023, enrolment was a choice between enrolling with ONE, TWO or THREE appointments included, the majority (70%) opting for the TWO appointment enrolment (around 10% opted for ONE and 20% for THREE appointments). From October 2023, enrolment was simplified so that everyone enrolling could do so with ONE 1-1 appointment, with the option to add additional 1-1 appointments as they progressed deeper in to the course.

What had become clear over those 4 years, was that up to TWO appointments suit some 90% of Users. A small proportion (around 5%) had needed a third appointment. Those that enrol with a mid-Category 3 Tinnitus Functional Index (TFI) where their tinnitus is "a moderate problem" needed just the one appointment.

During 2025....

It has become clear during 2025 that since CBT4T was last configured as CBT4T 2025 (December 2024) in to the four-element framework, CBT4T with ONE appointment is suitable for at least 80% of enrolments. Other enhancements have also contributed, particularly the weekly SITREP and the new CBT4T Substack (from April 2025). Prior to 2025, CBT4T with TWO appointments was the recommended option.

Due to this not insignificant change, CBT4T 2025-26 enrolments from October 2025 offer appointments as an addition to enrolment, rather than being included in the enrolment per se. This serves to simplify the enrolment process considerably too: ONE ENROLMENT then add your appointments separately.

Adding 1-1 appointments

You can add either ONE or TWO appointments when you enrol, or add them later. A first appointment is due at the end of Stage 4, and can be booked and paid for then.

A normal consultation is 60-90 minutes with a fee of £175, so make full use of the CBT4T rate:
