When you reach the final section of Stage 4 in CBT4T, you are prompted to repeat TWO of the clinical measures that you first completed on enrolment at Stage 1. The two measures are:
Tinnitus Functional Index (TFI)
Anxiety/stress Symptoms (ASQ)
When you complete each of these measures, a copy of your completed questionnaire measure is emailed to you by return email, so do check your email inbox, including Junk or Spam folders if it fails to reach your main inbox.
When I receive your TFI and ASQ, I will calculate each of them and send you an email with the results, comparing your Stage 4 results with those you completed at Stage 1.
I will also include a reserved date and time for your Zoom appointment – usually for the following week – and ask you to confirm its suitability OR, if the date and time is not convenient, we can re-arrange via email.
Your second and third appointments can be booked at any point after the first post-Stage 4 appointment. Although your enrolment on the programme extends to how ever long you want access to it, all appointments must be arranged within one year of enrolment.
